From high school health classes to university pharmacology programs, educators are increasingly asked to teach about cannabis. Here's an evidence-based curriculum framework that works.

Cannabis is now legal for adult use in nearly half of U.S. states, yet most educational institutions — from high schools to universities — lack coherent, evidence-based curricula for teaching about it. This gap has real consequences: young people are making decisions about cannabis use without accurate information, healthcare providers are entering practice without adequate cannabis pharmacology training, and the public discourse about cannabis policy is dominated by misinformation from both prohibition advocates and legalization proponents.

The foundational principle for cannabis education at any level is scientific accuracy. Cannabis education has historically been dominated by two equally problematic approaches: "Just Say No" abstinence messaging that ignores the evidence base and destroys credibility with students who know it's inaccurate, and pro-cannabis advocacy that minimizes real risks. Effective cannabis education presents the evidence honestly — including both the documented benefits and the documented risks — and trusts students to make informed decisions.

For high school health education, the most important topics are: adolescent brain development and why cannabis poses greater risks for young people than adults; the difference between occasional use and problematic use; the specific risks of high-potency products; the legal consequences of underage use; and harm reduction strategies for students who choose to use despite the risks. The evidence that cannabis use during adolescence is associated with cognitive impairment, increased risk of psychosis, and lower educational attainment is robust and should be presented clearly and without exaggeration.

The endocannabinoid system is the conceptual foundation for all cannabis science education. Students who understand that the body has its own cannabinoid system — with endogenous cannabinoids (anandamide, 2-AG), cannabinoid receptors (CB1, CB2), and metabolic enzymes — can understand why cannabis affects the brain and body the way it does. This framework also explains why cannabis has medical applications (it modulates a system that regulates pain, mood, appetite, and memory) and why it has risks (it disrupts normal endocannabinoid signaling).

University-level cannabis education is increasingly offered across multiple disciplines. Pharmacology and pharmacy programs should cover: cannabinoid pharmacokinetics and pharmacodynamics, drug interactions through the CYP450 system, the evidence base for medical applications, and the pharmacology of cannabis use disorder. Public health programs should cover: epidemiology of cannabis use, population-level health impacts, harm reduction frameworks, and the public health implications of legalization. Business programs should cover: the cannabis industry structure, regulatory environment, and the unique financial challenges of operating in a federally illegal industry.

Medical and nursing education has been slow to incorporate cannabis pharmacology, despite the fact that a significant percentage of patients in legal states use cannabis and that healthcare providers are frequently asked about it. A 2017 survey found that only 9% of medical schools had any cannabis content in their curricula. This is changing, but slowly. Medical educators should advocate for cannabis pharmacology content in pre-clinical curricula, and continuing medical education programs should offer cannabis-specific training for practicing clinicians.

The challenge of teaching about a federally illegal substance in federally funded institutions is real but navigable. The key distinction is between teaching about cannabis — which is clearly protected academic activity — and facilitating cannabis use, which is not. Educators can teach cannabis pharmacology, discuss the evidence base for medical applications, analyze cannabis policy, and even conduct cannabis research (with appropriate DEA registration) without violating federal law or institutional policies.

Practical resources for cannabis educators: the National Academies of Sciences, Engineering, and Medicine's 2017 report "The Health Effects of Cannabis and Cannabinoids" is the most comprehensive and authoritative review of the cannabis evidence base and is freely available online. The Cannabis Regulators Association (CANNRA) publishes regulatory resources. The Society of Cannabis Clinicians provides clinical education resources. PubMed contains thousands of peer-reviewed cannabis research articles. Building a cannabis curriculum from primary sources rather than advocacy materials from either side of the debate produces the most credible and effective educational content.

Assessment of cannabis education effectiveness should go beyond knowledge tests to measure attitudes, intentions, and ultimately behavior. The most effective cannabis education programs show changes in students' ability to critically evaluate cannabis-related claims, their understanding of risk factors, and their intentions to use cannabis responsibly if they choose to use at all. Programs that show no change in these outcomes — regardless of how much content they deliver — should be revised.
