Cannabidiol

The most researched non-psychoactive cannabinoid — with an FDA-approved medication and thousands of clinical studies.

CBD is the second most abundant cannabinoid in cannabis and the dominant compound in hemp. Non-psychoactive and well-tolerated, it has demonstrated efficacy for epilepsy, anxiety, inflammation, and pain — with an FDA-approved pharmaceutical (Epidiolex) for rare seizure disorders.

What is CBD?

Cannabidiol (CBD) is the most extensively researched non-psychoactive cannabinoid and has emerged as one of the most significant therapeutic compounds discovered in the 20th century. Unlike THC, CBD does not produce intoxication and is generally well-tolerated across a wide dose range.

CBD's mechanism of action is complex and multifaceted. Rather than binding directly to CB1 or CB2 receptors with high affinity, CBD acts as a negative allosteric modulator of CB1 receptors (reducing THC's psychoactive effects), an agonist at serotonin 5-HT1A receptors (anxiolytic effects), a TRPV1 channel activator (pain and inflammation), and an inhibitor of adenosine reuptake (anti-inflammatory). This multi-target pharmacology explains its broad therapeutic profile.

The landmark approval of Epidiolex (pharmaceutical-grade CBD) by the FDA in 2018 for Dravet syndrome and Lennox-Gastaut syndrome — two severe, treatment-resistant epilepsy syndromes — marked a turning point for cannabis medicine. It was the first plant-derived cannabis medicine approved by the FDA and provided definitive clinical evidence of CBD's anticonvulsant properties.

Product quality is the defining challenge in the CBD market. The 2019 FDA analysis found that 26% of CBD products contained significantly less CBD than labeled, and 21% contained more. Batch-level Certificates of Analysis from ISO 17025 accredited laboratories are the only reliable way to verify CBD content and confirm THC levels.

Full spectrum CBD — which preserves the complete cannabinoid and terpene profile including trace THC (≤0.3%) — is increasingly preferred over CBD isolate for most therapeutic applications. The entourage effect produces superior outcomes in multiple studies. Broad spectrum CBD (THC non-detect) offers a middle ground for those requiring verified zero THC.

Federally Legal

Hemp-derived CBD (≤0.3% THC) is federally legal in the US under the 2018 Farm Bill. Cannabis-derived CBD remains federally illegal but is legal in states with medical/recreational cannabis programs.

Key Facts

  • First isolated by Roger Adams in 1940
  • FDA-approved as Epidiolex for epilepsy (2018)
  • Non-psychoactive — does not produce a "high"
  • Negative allosteric modulator of CB1 receptors
  • Agonist at 5-HT1A serotonin receptors
  • Inhibits CYP450 enzymes — significant drug interactions
  • Hemp-derived CBD federally legal since 2018 Farm Bill

Important Warnings

  • Significant drug interactions via CYP450 enzyme inhibition
  • May cause liver enzyme elevation at high doses (Epidiolex data)
  • Full spectrum products may cause positive drug tests
  • Quality varies enormously — always verify with COA
  • Not FDA-approved for most uses (only epilepsy)
  • Consult physician before use with prescription medications
