Epilepsy & Seizures

The only FDA-approved cannabis-derived drug is for epilepsy. CBD reduces seizures by up to 50%.

Epidiolex — a purified CBD oral solution — is the first FDA-approved cannabis-derived medication, approved for Dravet syndrome and Lennox-Gastaut syndrome. Clinical trials showed 39–50% reductions in seizure frequency.

About Epilepsy & Seizures

Epilepsy affects 3.4 million Americans and is characterized by recurrent seizures caused by abnormal electrical activity in the brain. Approximately 30% of epilepsy patients have drug-resistant epilepsy — their seizures do not respond to available medications.

CBD's anticonvulsant properties were first widely publicized through the story of Charlotte Figi, a child with Dravet syndrome whose severe seizures were dramatically reduced by a high-CBD cannabis extract. This case catalyzed the modern medical cannabis movement and ultimately led to FDA approval of Epidiolex in 2018.

Epidiolex (purified CBD) is the first and only FDA-approved cannabis-derived medication. Clinical trials demonstrated 39% reduction in seizures for Lennox-Gastaut syndrome and 50% reduction for Dravet syndrome compared to placebo — remarkable results for conditions that are notoriously treatment-resistant.

The mechanism of CBD's anticonvulsant action is not fully understood but involves multiple pathways: modulation of sodium and calcium channels, enhancement of GABA inhibitory signaling, and reduction of neuroinflammation. Notably, CBD's anticonvulsant effects appear to be independent of CB1 receptor activation.

How Cannabis Helps

CBD modulates multiple ion channels (sodium, calcium, potassium) involved in neuronal excitability. It enhances GABA-mediated inhibitory signaling and reduces glutamate excitatory activity. CBD also reduces neuroinflammation through TRPV1 and GPR55 receptor modulation. These mechanisms are distinct from most antiepileptic drugs, making CBD effective in drug-resistant cases.

Key Facts

  • Epidiolex is the first FDA-approved cannabis-derived drug — approved for epilepsy
  • CBD reduced seizures by 39–50% in clinical trials for Dravet and Lennox-Gastaut
  • CBD's anticonvulsant mechanism is independent of CB1 receptors
  • 30% of epilepsy patients have drug-resistant epilepsy — CBD offers new hope
  • CBD interacts with several antiepileptic drugs — neurologist supervision required
  • Charlotte Figi's story with Dravet syndrome catalyzed the modern CBD movement

Important Warnings

  • CBD has significant drug interactions with antiepileptic medications — always work with a neurologist.
  • Never stop seizure medications abruptly to switch to cannabis.
  • THC can lower seizure threshold in some patients — use CBD-only products.
  • Liver enzyme elevations have been reported with high-dose CBD — monitor liver function.
