About Nausea & Vomiting

Nausea and vomiting are among the most debilitating symptoms patients face, whether from chemotherapy, pregnancy (hyperemesis gravidarum), gastroparesis, or other conditions. Cannabis has one of its strongest evidence bases for nausea โ€” the FDA approved synthetic THC (dronabinol) as an antiemetic in 1985.

THC suppresses nausea through CB1 receptors in the dorsal vagal complex and nucleus tractus solitarius โ€” the brainstem's vomiting center. This mechanism is distinct from most antiemetics (which target serotonin or dopamine receptors), making cannabis effective even when other antiemetics fail.

CBD also has antiemetic properties through 5-HT1A receptor activation, which suppresses the serotonin-mediated nausea pathway. CBD is particularly effective for anticipatory nausea โ€” the conditioned nausea that develops before chemotherapy โ€” which does not respond well to standard antiemetics.

An important consideration: while cannabis is highly effective for nausea, cannabinoid hyperemesis syndrome (CHS) โ€” a paradoxical condition causing severe vomiting in heavy cannabis users โ€” is increasingly recognized. CHS is distinct from typical nausea and requires cannabis cessation, not more cannabis.

How Cannabis Helps

THC activates CB1 receptors in the dorsal vagal complex (brainstem vomiting center) to directly suppress the emetic reflex. CBD activates 5-HT1A receptors to reduce serotonin-mediated nausea, particularly effective for anticipatory nausea. Both cannabinoids reduce gastric motility and inflammation that contribute to nausea. The combination of THC and CBD is more effective than either alone for most nausea types.
