Insomnia & Sleep Disorders

THC reduces sleep onset time. CBN is the most sedating cannabinoid. CBD improves sleep quality without sedation.

Insomnia affects 70 million Americans and is the second most common reason for medical cannabis use. Cannabis reduces sleep onset time, increases deep sleep, and reduces nighttime awakenings โ€” though long-term use requires careful management.

About Insomnia & Sleep Disorders

Insomnia is the most prevalent sleep disorder, affecting 30% of adults with chronic symptoms in 10โ€“15%. It is the second most common reason patients seek medical cannabis after chronic pain, and surveys consistently show 70โ€“80% of cannabis users report improved sleep.

Cannabis affects sleep architecture in complex ways. THC reduces REM sleep (the dreaming stage) and increases slow-wave (deep) sleep. This is beneficial for most insomnia patients โ€” less REM means fewer nightmares and more restorative deep sleep. However, REM suppression can cause vivid dreams when cannabis is stopped (REM rebound).

CBN (cannabinol) has emerged as the most sedating cannabinoid. While early research attributed CBN's sedation to THC contamination, more recent studies suggest CBN has independent sedating properties, particularly when combined with THC. CBN is increasingly marketed specifically for sleep.

CBD's relationship with sleep is dose-dependent and paradoxical: low doses (15โ€“25mg) can be mildly alerting, while high doses (150โ€“600mg) are sedating. CBD improves sleep quality primarily by reducing anxiety and pain that disrupt sleep, rather than through direct sedation.

Common Symptoms

Difficulty falling asleep (sleep onset insomnia)
Frequent nighttime awakenings
Early morning awakening
Non-restorative sleep
Daytime fatigue and impairment
Difficulty concentrating
Mood disturbances from poor sleep

How Cannabis Helps

THC binds CB1 receptors in the hypothalamus and brainstem to suppress REM sleep and increase slow-wave (deep) sleep, reducing sleep onset time by 30โ€“45 minutes. CBN acts on CB1 receptors with additional GABA-A modulation for sedation. CBD reduces sleep-disrupting anxiety and pain without direct sedation at typical doses. The endocannabinoid system regulates circadian rhythm through CB1 receptor modulation of the suprachiasmatic nucleus.

Key Facts

  • Insomnia is the #2 reason for medical cannabis authorization after chronic pain
  • THC reduces sleep onset time by 30โ€“45 minutes on average
  • CBN is the most sedating cannabinoid โ€” increasingly used in sleep formulations
  • CBD improves sleep quality primarily by reducing anxiety and pain
  • 75% of cannabis users report improved sleep quality
  • Nightly use can cause tolerance and dependence โ€” use lowest effective dose

Important Warnings

  • Nightly cannabis use can lead to psychological dependence โ€” take regular breaks.
  • REM suppression from THC can cause vivid dreams when stopping (REM rebound).
  • Morning grogginess ("cannabis hangover") is common with high doses โ€” start low.
  • Cannabis can worsen sleep apnea โ€” consult a sleep specialist if you snore heavily.
